GENETELIA EKSTERNA

Pudenda sama dengan Vulva dan organ reproduksi yang terlihat dari luar:

Monspubis,Labia mayora, labia minora, klitoris, VestibulaPerineum, Vsagina.


Mons Pubis
Sinonim : mons veneris;
Bagian yang dilapisi lemak, terletak di atas simfisis pubis;
Bagian kulit dari mons pubis ditumbuhi oleh bulu-bulu à
bulu pubis (pubic hair);

LABIA MAYORA
Homolog dengan skrotum pada laki-laki;
Ukuran labia mayora pada wanita dewasa à panjang 7- 8 cm,
lebar 2 – 3 cm, tebal 1 – 1,5 cm;
Pada anak-anak dan nullipara à kedua labia mayora sangat
berdekatan, pada wanita multipara lebih terbuka;
Bagian atas dimulai dari mons pubis, bagian bawah berakhir
di perineum posterior membentuk commisura posterior;
Di bagian dalam labia mayora banyak terdapat glandula
sebasea à menjaga kelembaban di sebelah dalam labia
mayora;
LABIA MINORA
Pada wanita nullipara, labia minora tidak terlihat à
terlindungi oleh labia mayora, pada wanita multipara à
labia minora menonjol keluar melewati labia mayora;
Setiap labia minora terdiri dari suatu jaringan tipis yang
lembab dan berwarna kemerahan;
Bagian atas labia minora akan bersatu membentuk
preputium dan frenulum clitoridis, sementara bagian
bawahnya akan bersatu membentuk fourchette
CLITORIS
Merupakan organ erogenik pada wanita;
Homolog dengan penis pada pria;
Terdiri dari glans, corpus dan 2 buah crura, dengan
panjang rata-rata tidak melebihi 2 cm;
Glans clitoridis dipenuhi oleh ujung-ujung syaraf yang
merupakan mediator sensasi erotis

VESTIBULA
Berbentuk seperti buah almond, dibatasi oleh labia minora,
memanjang mulai dari clitoris hingga fourchette;
Pada vestibula terdapat 6 buah lubang, yaitu orifisium
urethra eksterna, introitus vagina, 2 buah muara kelenjar
Bartholini, dan 2 buah muara kelenjar paraurethral;
Kelenjar bartholini berfungsi untuk mensekresikan cairan
mukoid ketika terjadi rangsangan seksual;
Kelenjar bartholini juga menghalangi masuknya bakteri
Neisseria gonorhoeae maupun bakteri-bakteri patogen
lainnya;
HYMEN (SELAPUT DARA)
Terdiri dari jaringan ikat kolagen dan elastik;
Pada wanita dewasa, ketebalan hymen sangat bervariasi dan
individual, diameter pembukaannya juga bervariasi mulai
dari yang sebesar ujung jarum hingga yang dapat dilewati
dengan mudah oleh 2 jari orang dewasa;
Bentuk hymen tidak dapat digunakan untuk menilai apakah
wanita bersangkutan telah memulai aktivitas seksual atau
tidak;
Saat melakukan koitus pertama sekali à dapat terjadi
robekan, biasanya pada bagian posterior;

VAGINA
Merupakan suatu struktur muskulomembranosa yang
memanjang mulai dari vulva dan berakhir di uterus;
Panjang dinding anterior vagina : 6 – 8 cm, panjang dinding
posterior vagina : 7 – 10 cm;
Pada wanita nullipara, dinding dalam vagina dipenuhi oleh
rugae vagina, sementara pada wanita multipara atau yang
sudah menopause, rugae menipis bahkan menghilang;
PERINEUM
Struktur yang membatasi antara vagina dan anus;
Dibatasi oleh otot-otot à muskulus levator ani dan
muskulus coccygeus;
Otot-otot berfungsi untuk menjaga kerja dari sphincter ani;
Penting diperhatikan ketika melakukan episiotomi;
GENITALIA INTERNA
UTERUS
TUBA FALOPII
OVARIUM
UTERUS
Pada wanita tidak hamil, uterus terletak di dalam kavum
pelvis, di bagian anterior dibatasi oleh kandung kemih dan
di bagian posterior dibatasi oleh rektum;
Uterus terdiri atas bagian fundus, korpus dan serviks;
Lapisan-lapisan uterus terdiri atas endometrium,
myometrium dan lapisan serosa;
Dalam keadaan tidak hamil, panjang uterus 6 – 8 cm pada
wanita nullipara dan 9 -10 cm pada wanita multipara
UTERUS
Berat uterus 50 – 70 gram pada wanita yang tidak hamil) dan
> 80 gram pada wanita hamil;
Uterus dapat menahan beban hingga 5 liter;
Serviks = porsio à bagian dari uterus yang masuk ke dalam
liang vagina à memiliki arti penting dalam hal kanker serviks
Posisi uterus dipertahankan oleh ligamentum-ligamentum :
ligamentum latum, ligamentum rotundum, ligamentum
infundibulopelvikum, ligamentum ovarii-proprii, ligamentum
uterosakralis;
UTERUS
Vaskularisasi uterus berasal dari arteri uterina yang
merupakan cabang utama dari arteri illiaka interna (arteri
hipogastrika interna);

Kira-kira 2 cm di sebelah lateral dari serviks, arteri uterina
dan vena uterina bersilangan dengan ureter à hati-hati
pada prosedur pembedahan à sering ter-klem atau
terpotong;
Tuba falopii
Panjangnya bervariasi : 8 – 14 cm;
Terdiri dari 4 bagian yaitu : pars interstitialis, pars isthmica;
pars ampullaris dan infundibulum tubae (fimbriae);
Pars interstitialis à merupakan bagian dari tuba yang
memasuki kavum uteri;
Pars isthmica à bagian pertengahan dari tuba dan
merupakan bagian yang paling sempit dengan diameter
2 – 3 mm;
Tuba falopii
Pars ampullaris à bagian tuba yang paling lebar dengan
diameter 5 – 8 mm;
Infundibulum tubae (fimbriae) à muara dari tuba fallopii
berfungsi untuk menangkap sel telur (ovum) yang dilepas-
kan oleh ovarium;
Mukosa tuba fallopii dilapisi oleh sel kolumnar yang
bersilia;
OVARIUM
Wanita umumnya memiliki dua buah ovarium, kanan dan
kiri;
Ukuran ovarium : panjang 2,5 – 5 cm, lebar 1,5- 3 cm dan
tebal 0,6 – 1,5 cm;
Ovarium merupakan penghasil sel telur, dimana satu atau
dua sel telur (ovum) akan matang setiap bulannya à
follikel de Graff;
Ketika dilahirkan, wanita memiliki cadangan ovum sebanyak
100.000 buah di dalam ovariumnya, bila habis à menopause

Aim: Introduce Body Temperature

•Objectives: At the end of this session students should be able to:

•Discuss the reasons why thermal regulation is important

•Describe the methods of heat gain and heat loss

•Without thermoregulation to facilitate exercise the body would overheat and the effect would result in death. Our core body temperature can drop 10 degrees and the body can still survive, but a core increase of just 5 degrees is all we can tolerate.

•Many athletes have died of heat stress for this reason. It is important to know how to cool off in hot weather by understanding thermoregulation and the best ways to make it work to your advantage.

Why is maintenance of body temperature important?

TEMPERATURE & METABOLSIM

•Temperature affects metabolism (and Ps) of organisms by affecting the speeds of chemical reactions and the effectiveness of enzymes.

Thermal Conditions

• Humans are homeothermic.

• A constant internal temperature of 36.1 to 37.80C.

• Body heat is transferred by conduction, evaporation, convection and radiation.

Factors That Affect Heat Gain

•BMR, muscular activity, hormones, thermic effect of food, postural changes, and environment. (Total metabolic rate can increase 3 to 5 times from shivering and 20 to 25 times during sustained vigorous exercise in aerobically fit individuals.

•Heat is conserved when blood is shunted into the cranial, thoracic and abdominal cavities and portions of muscle mass for insulation from the shell.

Factors That Affect Heat Loss

•Radiation, conduction, convection, and evaporation, which is most important.

•The body is cooled when internal heat buildup causes the blood vessels to dilate and direct warm blood to the shell.

Hypothalamic Regulation of
Core Temperature

•The hypothalamus contains the central coordination center for temperature regulation. It initiates the responses that keep the body from overheating or overcooling

•Heat-regulating mechanisms are activated by either:

•Thermal receptors in the skin or,

•Temperature changes in the blood

•Free nerve endings in the skin respond to heat and cold and relay the senses to the hypothalamus and cerebral cortex.

Conduction

• A direct transfer of heat through a liquid, solid or gas from one molecule to another.

• Although most of the body heat is transported to the shell by circulation, a small amount continually moves by conduction through the deep tissues to the cooler surface

Radiation

• Objects continually emit electromagnetic heat waves. Because our bodies are usually warmer than the environment, the net exchange of radiant heat energy is through the air to the solid, cooler objects in the environment.

Convection

• The effectiveness of heat loss by conduction depends on how rapidly the air or (water) near the body is exchanged once it becomes warmed.

Evaporation

• Heat is continually transferred to the environment as water is vaporised from the respiratory passage and skin surface.

• For each litre of water that vaporises, 580 kcals are extracted from the body and transferred to the environment.
